Job summary

University of Wisconsin–Madison’s Division of Extension, Health & Well Being Institute, Covering Wisconsin is seeking an Access to Care Coordinator to connect Wisconsin residents with preventive healthcare services and to refine our CWI approaches. The role involves meeting with communities, coordinating with partner agencies, and presenting results to stakeholders.

The position emphasizes collaboration, relationships, and the ability to navigate education and health services to improve access

Qualifications

• Demonstrated ability to effectively work with people from a wide range of backgrounds and perspectives.
• Demonstrated ability to work in a fast-paced environment while prioritizing and managing multiple initiatives at one time.
• Demonstrated experience with building diverse, collaborative partnerships.
• Existing knowledge of common access to care barriers. This includes awareness of potential mitigation strategies, especially for rural communities and other populations most at risk for lacking a medical home or primary care provider or underuse of preventive services.

Responsibilities

• Directs the implementation of program policies and procedures.
• Schedules day-to-day activities for program staff and student and community volunteers.
• Schedules and secures logistics and resources in support of an outreach program.
• Researches, develops, and facilitates outreach program content and materials.
• Identifies, promotes, and maintains internal and external partnerships to promote the outreach program.
• Identifies opportunities for program improvement or new outreach program development.
