Qualifikationen

Aufgaben

• Coordinate patient care with the Care Team and non-adherence outreach.
• Engage patients needing services and follow up on requests.
• Respond to eRecord and e-mail requests from clinical staff in a timely manner.
• Provide patient-centered service and build trust within the health system.
• Identify resources to overcome barriers to care, including transportation and scheduling.
• Provide health education and support for insurance navigation through NYS Health Exchange.
• Maintain documentation of patient encounters and reporting per standards.
• Contribute to team meetings and process improvement initiatives.
