Job summary

Enhabit Home Health & Hospice is seeking an intake coordinator to support patient referrals and coordinate transitions to services. This role emphasizes service excellence, regulatory compliance, and collaboration with branch and sales teams to ensure seamless patient care.

A licensed LVN/LPN, or nursing graduate, with at least two years in healthcare (one in home health or pediatrics), strong customer service, Medicare knowledge, and basic technology skills is required.

Qualifications

• Must have a high school diploma or equivalent.
• At least 2 years in health care, with 1 year in home health or pediatrics.
• Experience in customer service.
• Knowledge of Medicare guidelines for home health/hospice.
• Basic tech skills, including use of a mobile device.

Responsibilities

• Coordinate patient referrals and serve as a resource for referral sources.
• Collaborate with branch and sales teams to coordinate transitions to services.
• Apply and demonstrate knowledge of regulatory guidelines and governing processes.
