Qualifications

• RN licensure with valid state credentials.
• Nursing Diploma/Associate's required; Bachelor's degree preferred.

Responsibilities

• Promotes patient and family centered care in a healing environment.
• Educates patients and families on illness management and home care needs.
• Participates in patient safety, quality and regulatory compliance activities.
• Develops, implements, and documents care plans with the interprofessional team.
• Assesses patient needs and communicates the plan of care across the continuum.
