Qualifications

• Wisconsin social work licensure is required.
• Bachelor’s Degree in Social Work required; MSW preferred.
• Demonstrated knowledge of social work in healthcare settings.
• Excellent communication and human relations skills.
• Proficient in MS Office and EMR software.

Responsabilités

• Provide social work services to individuals or groups, including screening and psychosocial assessment.
• Discharge planning and determination of social work goals.
• Maximize patient coping, exercise professional judgment, and document activity.
• Coordinate with home care, palliative care, hospice and other facilities when appropriate.
• Advocate for patient involvement in the plan of care and coordinate with healthcare team.
• Educate patients/families on Advance Directives and assist with completion.
