Mayo Clinic in Minnesota is seeking a Care Team RN to support the Cardiology Outpatient Heart Failure Clinic, with coverage for Amyloid and Sarcoid Clinics. The RN provides direct patient care, pre-visit preparation, patient education and care transitions within the Mayo Nursing Care Model.

State licensure required; TN sponsorship available; BLS required. Schedule is 56–64 hours per pay period, with 8-hour daytime shifts, Monday through Friday.

Qualifications

• Graduate of a nursing program with RN licensure.
• A minimum of 1 year RN experience required.
• Current direct patient care experience and strong communication skills demanded.

Responsabilités

• Provide direct patient care in Cardiology Outpatient Heart Failure Clinic.
• Involve pre-visit preparation, patient/family education and care transitions.
• Collaborate with providers and Care Team for quality improvement and safe care.
