Mayo Clinic is seeking a Care Team RN to support the Cardiology Outpatient Practice, primarily in the Heart Failure Clinic, with additional coverage for Amyloid and Sarcoid Clinics. The RN delivers direct patient care, pre-visit preparation, and patient education, while coordinating with providers and other care team members within the Mayo Nursing Care Model.

The role requires active RN licensure, BLS clearance, and strong communication skills.

Formación

• One year of RN experience required.
• Current RN license by applicable state requirements.
• BLS (Basic Life Support) must be valid for 90 days post start date.
• Cardiac experience preferred.
• Experience working in a team environment.
• Computer skills required; EMR experience preferred.

Responsabilidades

• Provide direct patient care and education in the Heart Failure Clinic and related cardiology outpatient areas.
• Coordinate nursing care across the care team and transitions of care.
• Maintain high standards of nursing practice per Mayo Clinic guidelines and ANA standards.
• Demonstrate leadership in preceptor and quality improvement activities.
