Mayo Clinic in Minnesota is seeking a Care Team RN for the Cardiology Outpatient Heart Failure Clinic with coverage for Amyloid and Sarcoid Clinics. The RN provides direct patient care, pre-visit preparation, and patient/family education, coordinating with providers and the care team.

Licensed RNs with strong communication, teamwork and critical thinking are encouraged to apply. The role requires BLS, state RN licensure, and eligibility for TN sponsorship; VisaScreen or equivalent certification

Qualifications

• Graduate of a nursing program recognized by ACEN/CCNE/NLN CNEA.
• One year RN experience required.
• Current RN license and BLS certification required.

Responsibilities

• Provide pre-visit preparation, direct patient care, patient/family education and transitions of care.
• Collaborate with providers and care team for coordinated nursing practice.
• Lead nursing care planning, evaluation and communication across care settings.
• Support quality improvement efforts and participate in committees/projects.
