Qualifications

• B.S. or Pharm D. from an ACPE-accredited School of Pharmacy.
• Completion of an accredited PGY-1 Pharmacy Residency or fellowship.
• 3+ years of clinical pharmacy experience if PGY-2 is not completed.
• 5+ years of clinical pharmacy experience without residency or fellowship.
• Active Pharmacist license in good standing with the appropriate State Board.

Responsibilities

• Provide patient-centered care and integrated pharmacy services.
• Participate in the medication-use system providing comprehensive pharmaceutical care.
• Support centralized and decentralized medication-use systems.
