The role focuses on safe, effective, and prompt medication therapy within a decentralized practice setting, collaborating with physicians, nurses, and other hospital staff to deliver patient-centered pharmaceutical care. Candidates should have a B.S.

or Pharm D. from ACPE-accredited school, hospital clinical pharmacy experience, and Board Certification or equivalent residency experience;

Qualifications

• B.S. or Pharm D. from ACPE-accredited School of Pharmacy.
• Clinical pharmacy experience in a hospital setting.
• Completion of an accredited PGY-1 Pharmacy Residency or fellowship AND PGY-2 Specialty Residency or fellowship.
• OR PGY-1 Pharmacy residency or fellowship AND 3+ years clinical pharmacy experience (no PGY-2).
• OR 5+ years clinical pharmacy experience with no residency or fellowship.
• Board of Pharmacy Specialties certification (or within 1 year).
• Active Pharmacist license in good standing with the state board.
• Other state requirements – e.g. sterile compounding.
• ALS/ACLS certification.
• BLS certification must be obtained within 14 days of hire.
• Board Certification.

Responsibilities

• Provide safe, effective, and prompt medication therapy.
• Provide patient-centered care and integrated pharmacy services in a decentralized setting with clinicians.
• Support centralized and decentralized medication-use systems.
