Job summary

Banner Health in Phoenix is seeking a Patient Services Navigator to optimize the patient and family experience within hospital and ambulatory care settings. You will collaborate with providers and staff to improve care coordination, assist with scheduling, and support the patient journey from initial contact through follow-up appointments.

Qualifications

• Medical terminology knowledge required.
• Medical Assistant Training or CNA/PCT training.
• Two years of health care experience required.
• Excellent time management and customer service skills.

Responsibilities

• Researches concerns and provides resolutions to patients’ issues in collaboration with team.
• Supports patient-centered care program development and participates in patient satisfaction initiatives.
• Assists with administration of physician offices and outpatient facilities to meet goals.
• Maintains effective communication with all constituents inside and outside Banner Health.
• Coordinates with social services for lodging/transportation advice for patients traveling to Banner facilities.
• Helps coordinate multiple appointments using scheduling systems for cross-discipline communication.
• Assists patients and families at initial appointments and pre/post-surgery rounding.
• Participates in multidisciplinary tumor board coordination and related activities.
• Works independently to provide excellent patient care to both internal and external customers.
