St. Joseph’s/Candler Health System in Savannah, GA seeks a Nurse Practitioner (Cardiology) to assess patients in an office setting, obtain histories and physicals on new patients, educate patients and document progress notes.

You will consult with physicians on complex cases, prescribe medications, order labs and radiology as needed, and coordinate with specialists and allied teams to implement care plans.

Qualifikationen

• Educates patients about their medical condition. Comprehensive review of the patient's medical history is performed and documented.
• Prescribes medications that are necessary to control medical condition. Refers patient to specialist when necessary. Orders lab work, radiology studies as needed to better assess the patient's condition.
• Obtains a complete history and physical on new patients. Charts and dictates progress notes appropriately. Confers with physician on patients presenting with more complex problems.
• Clinical procedures are carried out based on clinical protocol. Informs patients about procedures prior to performing and communicates results to patients. Informs patients on provider instructions and any preparations.
• Refers patients to specialists if needed. Maintains liaison with other disciplines, such as social services, nutritional speech therapy, physical therapy, pastoral care, and pain management. Implements plan of care.
