Job summary

BJC Medical Group is seeking an Advanced Practice Provider to manage acute and chronic illnesses across Cardiology, Vascular Surgery, Orthopedics and Post-Acute settings in hospital, clinic and skilled nursing facilities. The role requires collaboration with physicians and entails patient rounding, care coordination and on-call responsibilities.

Ideal candidates hold an NP or PA license with nursing background, and have 2+ years of experience in specialty care.

Qualifications

• Master's degree in nursing required.
• Nurse Practitioner and RN licenses required.
• 2 years of experience preferred.
• Experience in specialty areas such as Cardiology, Vascular Surgery, Orthopedics and Post-Acute.

Responsibilities

• Manages health care of patient populations in hospital, SNF and clinic settings with procedures, rounding and prescriptions.
• Develops treatment plans and initiates management under the collaborating physician.
• On-call for inpatient services and ambulatory follow-up; supports medical management.
