Provider Requirements:

1. Title: Physician (MD or DO) in General Non-Invasive Cardiology

2. License: Oregon license preferred; candidates in process will be considered

3. Privileges: Not required; quick start possible

4. Holiday Coverage: New Year’s Day, Memorial Day, 4th of July, Labor Day, Thanksgiving, Christmas

5. Additional Notes:

   • Flexible scheduling can consider 4 days if a full week isn’t possible

   • Reopened due to previous provider cancellation

   • Preference for a single provider to maintain consistency
