Description

• Weekly Schedule: Monday to Friday, 8:00 a.m. to 4:30 p.m.

• Clinic Schedule: Three days per week in Chesterton, IN, and two days in Michigan City; alternating with three days at Legacy Hills Health Center in LaPorte, IN, and two days in Michigan City.

• Travel to outreach locations.

• 36 patient contact hours and 4 hours of administrative time per week.

• Call: 1:4 rotation evenings and weekends.

• Average outpatient volume: 16-20 patients per day.

• Average rounding volume: 10-15 patients per day.

• Support from Advanced Practice Providers in Stress Lab at the hospital.

• Opportunity to build a large, busy practice quickly.

• Telehealth services offered.
