Job summary

Veterans Health Administration (VA Connecticut) is seeking a Cardiology Advanced Practice Provider to support a rapidly expanding Structural Heart and LAAO program in West Haven. The role covers pre-, peri-, and post-procedural care, with longitudinal follow-up for high-acuity Veterans.

You will coordinate care with Valve/LAAO clinics, conduct comprehensive evaluations, perform risk stratification, and ensure timely imaging and follow-up.

Qualifications

• Provide direct patient care in Valve and LAAO clinics with comprehensive evaluations and risk stratification.
• Coordinate pre- and post-procedural care and follow-up imaging.
• Collaborate with RN triage and multiple clinical teams for high-acuity Veterans.

Responsibilities

• Provide direct patient care in Valve and LAAO clinics: evaluations and risk stratification.
• Perform pre-procedural preparation including imaging review and education.
• Support day-of-procedure activities and discharge planning.
• Manage post-procedural care and follow-up studies, including ECHO scheduling.
• Coordinate with VA and Yale teams for structural heart pathways.
• Review and triage complex consults; document care plans in CPRS.
