UVA Health's Culpeper Medical Center is seeking a full-time Advanced Practice Provider to support its growing cardiology program in an outpatient clinic setting. The APP will practice at the top of their license within a collaborative cardiology team.

Responsibilities

• General cardiology outpatient clinic visits
• Inpatient cardiology consults and rounding in collaboration with physicians
• Care coordination across inpatient and outpatient settings
• Evaluate, diagnose, and manage new and follow-up patients with General Cardiology and complex cardiovascular prevention needs.
• Perform cardiovascular assessments; order and interpret diagnostic testing; develop and implement individualized treatment plans.
• Manage cardiovascular risk factors and chronic conditions
• Manage general cardiovascular conditions such as hypertension, coronary artery disease, heart failure, arrhythmias, and valvular disorders.
• Provide patient and family education regarding disease processes, treatment options, procedural care, and long-term self-management.
• Coordinate diagnostic testing, referrals, and follow-up care.
• Document clinical encounters accurately and timely in the electronic medical record (EPIC).
