• Perform cardiovascular assessments; order and interpret tests; develop treatment plans.
• Manage cardiovascular risk factors and chronic conditions.
• Educate patients and families on disease processes and self-management.
• Coordinate diagnostic testing, referrals, and follow-up care.
• Document encounters in the EPIC electronic medical record.
