Job summary

University Hospitals Pain Management seeks an experienced Advanced Practice Provider for outpatient cardiology services at UH Parma Medical Center, with travel to Medina and Fairlawn locations. Schedule is days Mon-Fri 8:00 am-4:30 pm. APP will see new and established patients; cardiology experience preferred.

The role requires APP experience and Ohio license, with adherence to nursing board guidelines and UH bylaws.

Qualifications

• Ohio State Nursing Board Nurse Practitioner/Advanced Practice Provider scope of care.
• Cardiology experience preferred for outpatient follow-ups and new patient assessment.

Responsibilities

• See new and established patients in outpatient cardiology.
• Perform patient assessments and determine diagnoses.
• Develop and implement care plans based on care pathways and standards of care.
• Collaborate with interdisciplinary team to provide patient care.
• Support staff development and education.
• Demonstrate leadership and professional development.
