Responsibilities

• Assesses and analyzes the health status of patients on the unit.
• Performs ongoing assessments and documents findings.
• Initiates discharge planning with initial admission assessment.
• Plans patient care in collaboration with the patient, family, and health care team.
• Transcribes and directs the implementation of physician orders; provides patient education.
• Directs care team members to deliver quality care.
• Updates plan of care based on patient assessments and time frames.
• Communicates and documents all patient care, including assessments, plan, interventions, and follow-up.
