Job summary

Qualifications

Responsibilities

• Assess and monitor patient health status on the telemetry unit.
• Perform ongoing assessments and document findings per policy.
• Plan and implement patient care in collaboration with the health care team.
• Discharge planning and patient/family education.
• Communicate plan of care during shift reports and with the care team.
• Direct and coordinate care activities within scope of practice.
