Job summary

Virginia Mason Franciscan Health is seeking a HART Patient Care Coordinator RN to provide expert nursing leadership and meticulous coordination for complex cardiac arrhythmia patients.

You will manage patient pathways, educate patients and families on treatment plans, schedule advanced cardiac procedures, and collaborate with multidisciplinary teams to optimize outcomes for heart rhythm disorders.

Qualifications

• Graduation from an accredited school of nursing.
• Two years of related nursing work experience in an acute clinical setting within the last five years.

Responsibilities

• Interviews and evaluates patients for hospice eligibility per Medicare criteria.
• Explains hospice services and eligibility criteria to patient/family.
• Elicits clinical information from patient/family regarding health status and readiness for hospice care.
