Apex Home Health, Hospice & Home Care in St. Louis is seeking an experienced registered nurse to provide direct nursing care to adults in an outpatient cardiac wellness setting.

You will assess needs, develop and document care plans, educate patients and families, coordinate with the interprofessional team, and deliver compassionate, patient-centered care while monitoring patient status.
