Job summary

Lincare Holdings is seeking a Registered Nurse to oversee the cardiovascular rehabilitation program, providing comprehensive nursing care for cardiopulmonary patients in a hospital setting. The role emphasizes assessment, planning, implementation, and evaluation of patient care in coordination with physicians.

The incumbent will supervise exercise sessions with cardiac patients, monitor vitals, and educate patients and families on risk factors and lifestyle modifications.

Qualifications

• Graduate from an accredited school of professional nursing, Required
• One year in a hospital-based cardiology related patient care unit, Required
• Critical care experience, Required
• Monitoring and interpretation of electrocardiograms, Required

Responsibilities

• Oversees the direction, coordination, and evaluation within the cardiovascular rehabilitation program
• Administers nursing care to cardiopulmonary patients, including assessment, planning, implementation, and evaluation
• Supervises patients during exercise sessions, monitoring vital signs to ensure safety
• Provides patient/family education and counseling for risk factors and lifestyle modifications
• Maintains accurate and detailed records of patient progress and care plans
• Initiates emergency procedures according to policy
• Facilitates phase I to phase II transition for cardiac rehab and follows up as needed

Job description

This employee assumes responsibility and accountability for assigned patients (adult and geriatric).

• Works with supervisionwithin policy and program guidelines
• Provides professional nursing care, incorporating the nursing process, the
  environment, technical equipment, and other health care members
• Provides input for program maintenance anddevelopment
• Works independently with limited direct supervision using the company policy and professionaljudgment

JOB FUNCTIONS

• Oversees the direction, coordination, and evaluation within the cardiovascular rehabilitation program
• Administers nursing care to cardiopulmonary patients
  • includes assessment, planning, implementation, andevaluation of the patient's plan of care in coordination with the physician
• Demonstrates awareness of legal issues in all aspects of patient care and unit function
  • strives to managersituations in a reduced risk manner and promote patient safety
• Solves issues with staff, physicians, patients, families, and ancillary departments
• Assumes job responsibilities of other team members during their absence
• Assesses the cardiovascular inpatient population to provide educational plans
• Identifies individual risk factors associated with cardiac and pulmonary disease, and assists with risk stratification
• Identifies the need for reevaluation of each patient if the patient's condition changes and warrants intervention
• Supervises patients during exercise sessions, monitoring vital signs, including heart rate, blood pressure, andoxygen saturation levels to ensure safety during exercise sessions
• Provides patient/family education and counseling for identified risk factors and develops life-style modificationspertaining to cardiopulmonary disease and related health issues
• Maintains accurate and detailed records of the patients progress, treatments, and care plans in compliance withdepartmental and hospital policies for medical records
• Develops individualized educational plans of care based on patient's goals and expectations
• Evaluates, updates, and revises plan of care to facilitate achievement of planned and expected outcomes
• Initiates emergency procedures according to policy
• Reviews patient history and other medical record to develop a plan of care that will include behavior modificationthrough activity and diet intervention
• Facilitates phase I to phase II transition process for cardiac rehab
  • actively assigns patients to phase II whoqualify for the program
  • Provides follow-up phone calls as necessary
• Actively participates in the patient education and community based programs by organizing classes,communicating with speakers and participants, as well as providing actual classroom instruction
• Demonstrates competency in assessing emergent patient situations that require physician or emergency medicalinterventions

Education

• Graduate from an accredited school of professional nursing, Required
• BSN, Preferred

Work Experience

• One year in a hospital-based cardiology related patient careunit, Required
• Critical care, Required
• monitoring and interpretation of electrocardiograms, Required

Knowledge, Skills, and Abilities

• Extensive understanding of cardiac disease, anatomy and physiology, risk stratification, exercisephysiology, and diet and nutrition are essential

Licenses and Certifications

• Advanced Life Support (ALS) certification from the American Red Cross or American
  Heart Association certification, Required
• Licensed as a Registered Nurse in the state the services are provided, Required
