Job summary

Qualifications

• RN license required as per job description.
• Associate's degree in Nursing required.
• Bachelor's degree in Nursing preferred.

Responsibilities

• Advise patients on gains/risks of the program and obtain prerequisite written orders and informed consent.
• Develop individualized, result-oriented care plans with clients and referring physicians.
• Evaluate new cardiac patients to obtain medical history, identify risk factors and disease manifestations.
• Perform clerical/housekeeping duties and maintain equipment/supply inventories; may float to other sites.
• Assist in development of cardiac rehab policies and procedures.
• Respond to patient condition changes, alert physicians, and handle emergencies.
• Collect data, document patient outcomes, and track progress toward benchmarks.
• Document in Cerner/Series per insurance guidelines and post charges accurately.
• Conduct ongoing patient assessments (EKG, vitals, breath sounds) and progress rehab plans.
• Provide instruction on rehabilitation concepts and lifestyle modification.
• Motivate patients and adjust programs based on individual progress.
