An exciting opportunity has arisen to join the Cardiac Rehabilitation Team – ideal for those interested in exploring specialist practice.

This 12-month secondment (0.6 WTE) offers the chance to work as a specialist practitioner within a supportive team. An additional 0.4 WTE (funded by NHS England until 31st March 2025) is available if desired, allowing for a full-time role.

The post holder will be a key member of the multidisciplinary team within University Hospitals Bristol and Weston NHS Foundation Trust (UHBW) and the community. They will provide expert advice and support to patients and families across in-patient and out-patient settings, throughout the care pathway.

The role involves using clinical expertise, leadership, education, and management skills to ensure a high-quality, coordinated service. The post holder will work closely with hospital and community teams, contribute to education, audit and research, and lead discussions in MDT meetings. They will also line manage the Cardiac Rehab services across UHBW sites.

Main duties of the job

Support the Lead Practitioner to:

  • Plan and manage daily operational tasks.
  • Monitor team performance through appraisals, supervision, and regular meetings.
  • Provide clinical and managerial support, particularly to Band 6 practitioners, ensuring safe working practices.
  • Support staff in prioritising workloads based on clinical need.
  • Address immediate staffing issues, including sickness and absence.
  • Respond initially to complaints and queries, in line with UHBW NHS Trust procedures.
  • Maintain accurate records to support service evaluation and development.
  • Represent the service on relevant committees and project teams.
  • Assist in resource planning and service development with the Matron.
  • Contribute to strategic and business planning.
  • Support the development of evidence-based policies and procedures.
  • Promote quality improvement through audit and risk management.
  • Participate in setting and auditing professional standards.
  • Adhere to Trust policies and take part in annual performance reviews.

Detailed job description and main responsibilities

· Support the lead practitioner in establishing and delivering practitioner-led interventions to enhance services and effectively manage patients’ needs and expectations.

· Prescribe and review medications for therapeutic effectiveness (if a Non-Medical Prescriber or under Patient Group Directions), in accordance with evidence-based practice, national guidelines, and within scope of practice.

· Develop and maintain links with external organisations to provide additional patient support and informational resources.

· Adhere strictly to infection control guidelines and protocols.

· Act as a positive role model for the multidisciplinary team (MDT) by demonstrating strong leadership and clinical excellence.

· Work clinically as part of the cardiac rehabilitation team, sharing knowledge, expertise, and role modelling for junior staff, in line with the divisional approach.

· Provide clinical support and advice in wards and outpatient settings, identifying and implementing opportunities for quality improvement.

· Analyse and respond to complex clinical situations using specialist knowledge to assess options and develop effective solutions.
