Job summary

Qualifications

• 1 year of applicable clinical experience.

Responsibilities

• Develops and delivers individualized patient-centered treatment plans.
• Collaborates with Nursing and multidisciplinary team.
• Provides exercise leadership, instruction and prescription.
• Documents processes per policy and insurance requirements.
• Contributes to discharge planning and quality initiatives.
