Qualifications

• Degree in physical therapy, exercise science, or equivalent required.
• ACLS certification required; instructor options through SSH may be available.
• Experience with cardiac rehabilitation preferred; strong patient communication skills.

Responsibilities

• Perform evaluation and create individualized cardiac rehab plans.
• Conduct exercise sessions and monitor patient safety and progress.
• Provide patient education and counseling on risk factor reduction.
• Document patient progress in the electronic health record and care plans.
• Collaborate with multidisciplinary team and refer to appropriate specialists.
• Lead group exercise sessions and adapt plans for patients with complex needs.
• Coordinate with care teams to ensure timely discharge and follow-up.
