Job summary

Abrazo Arizona Heart Hospital in Phoenix, AZ, seeks a Cardiac Rehabilitation Therapist to implement and follow up outpatient rehab programs. You will provide personalized exercise plans for adults with cardiovascular disease.

You will monitor vitals, educate patients, and collaborate with cardiology teams to optimize recovery and health outcomes. Bachelor’s in Exercise Physiology and BLS required; ACLS preferred; 2–4 years in rehab preferred.

Qualifications

• Experience: Cardiac Rehabilitation internship or employment experience required.
• Education: Bachelor’s degree in Exercise Physiology or related field required.
• Certifications: BLS; ACLS, if required by the facility.
• Preferred: 2-4 years of Cardiac Rehabilitation experience.
• Certification: ACSM CEP or equivalent preferred.

Responsibilities

• Supervise and monitor outpatient cardiac rehab Phase 2 and 3 for adult patients.
• Monitor and assess blood pressure and EKG rhythm during exercise.
• Create individualized treatment plans: risk assessment, management, goals, exercise prescription.
• Process referrals from cardiology offices and hospitals.
• Explain cardiac medications, procedures and CAD process.
• Understand cardiovascular emergency procedures and medications.
• Promote patient relations and positive relationships.
• Provide weekly outpatient education and periodic inpatient education.
• Maintain program certification standards (BP, smoking cessation, psychosocial assessment).
