Mayo Clinic in MN is seeking a registered nurse to coordinate patient care, educate patients and families, and facilitate transitions of care. The RN will practice within the Mayo Clinic Nursing Professional Practice Model and contribute to quality improvement while mentoring colleagues.

Qualifications

• Graduate of a nursing program.
• Current RN license by applicable state requirements.
• Bachelor's degree in nursing preferred within five years from RN start date.
• One year of RN experience or Mayo Clinic RN/LPN experience preferred.

Responsabilités

• Coordinate nursing care, including direct patient care and patient/family education.
• Support professional nursing practice across practice settings and transitions of care.
