The ideal candidate will have an accredited nursing program, active RN license, BLS, and strong communication, teamwork, and decision-making skills to coordinate discharge planning and ensure patient safety.

Qualifications

• Registered Nurse license issued by the state of practice.
• Basic Life Support (BLS) for Healthcare Providers certification from the American Heart Association.
• Completion of an accredited or approved nursing program.

Responsibilities

• Use the nursing process to assess patient needs, plan and implement care, and involve patients and families.
• Anticipate and respond to significant changes in patient condition with ongoing assessment.
• Coordinate discharge planning using an interdisciplinary approach and evidence-based practice.
• Promote safety by identifying risks and reporting safety events promptly.
• Demonstrate communication and collaboration skills; lead and delegate to unlicensed staff.
