Job summary

The New Mexico Department of Health seeks a CARA Navigator to ensure compliance with CARA and develop plans of care for newborns exposed in-utero. You will build relationships with families, perform assessments, and coordinate with hospitals and state agencies to connect families with needed services.

Responsibilities include reviewing plans of care, conducting home visits, training providers on CARA requirements, and supporting data-driven reporting with culturally informed practices.

Qualifications

  • Associate degree in social work or related field and 1 year of relevant experience.
  • Experience with data entry, quality assurance, and data analysis.
  • Knowledge of culturally responsive practices and tribal communities.
  • Experience working independently and as part of a multidisciplinary team.
  • Experience with case management systems and electronic databases.
  • Ability to perform user acceptance testing and document testing results.

Responsibilities

  • Review every plan of care to ensure completeness and accurate insurance referrals.
  • Conduct comprehensive family assessments and revise plans as needed.
  • Perform home visits to support engagement and CARA compliance.
  • Contact Statewide Central Intake for safety concerns when required.
  • Assist in training providers on CARA law and care plan creation.
  • Provide outreach to target populations and offer CARA resources.
  • Lead CARA workgroup discussions and coordinate with other state agencies.
