Alliance Recruiting Resources is seeking an experienced Family Nurse Practitioner to join its primary care team on Cape Cod, Massachusetts. This role involves delivering patient-centered care with a strong emphasis on chronic disease management and preventive services.

Qualifications

• Minimum of 2 years of Nurse Practitioner experience.
• Experience in a community health center or similar setting strongly preferred.
• Ability to deliver comprehensive care including preventive services.

Responsibilities

• Serve as the primary care provider for an assigned patient panel.
• Deliver comprehensive care including preventive services, acute care, and chronic disease management.
• Conduct routine wellness visits including full histories, physical exams, screenings, and patient education.
• Coordinate and manage care, including referrals to specialists.

Education

Graduate of an accredited Family Nurse Practitioner program

Current Massachusetts Nurse Practitioner license
