Job summary

NPACE, a mission-driven healthcare organization, seeks an experienced Family Nurse Practitioner to join its primary care team in Hyannis, Massachusetts. You will deliver patient-centered care across the lifespan with a focus on prevention, chronic disease management, and acute care.

Qualifications

Graduate of an accredited Family Nurse Practitioner program.
Current Massachusetts NP license or eligible.
Board Certification as a Family Nurse Practitioner.
Current DEA, MCSR, and BLS certification.
Minimum of 2 years of Nurse Practitioner experience.
Proficiency with computers and EMR systems (EPIC/OCHIN experience preferred).
Experience in a community health center or similar setting strongly preferred.
Bilingual skills a plus (English/Portuguese/Spanish).

Responsibilities

Serve as the primary care provider for an assigned patient panel.
Deliver comprehensive care including preventive services, acute care, and chronic disease management.
Conduct routine wellness visits including full histories, physical exams, screening, and patient education.
Coordinate and manage care, including referrals to specialists, behavioral health, nutrition, and social services.
Contribute to evening, weekend, and holiday clinic coverage as needed.
