Job summary

Eskenazi Health in Indianapolis seeks a Clinical Oncology/Hematology Coordinator to coordinate a patient-centered care model for cancer and blood disorders. Qualifications

• Associate's degree in Nursing required; Bachelor's degree preferred.
• One year of clinical nursing experience required; three years preferred.
• Current Indiana license as an RN or eligibility for license in Indiana.

Responsibilities

• Evaluates effectiveness of interventions and revises care plan with the team.
• Schedules new oncology/hematology patients using scheduling software.
• Main contact for referring physicians and patients/families; document communications in medical records.
• Coordinates patient scheduling, prescription refills, and test result communications.
• Obtains pre-certification/pre-authorization for services and medications.
• Educates patients and families across the continuum of care including pain, chemotherapy, palliative care and hospice.
