Job summary

Holy Cross Hospital in Fort Lauderdale, part of Trinity Health, is seeking a Patient Navigator to coordinate care and support patients through intensive case management within the Community Health and Well-Being department.

Graduate of an accredited nursing program with FL RN license is required; BSN preferred. Strong collaboration with clinicians and community partners will be essential to link patients to financial aid and appropriate care pathways.

Qualifications

• Graduate of an accredited school of nursing is required.
• FL RN license is required.

Responsibilities

• Plans and implements programs to meet the needs of the Community Health and Well-Being department.
• Collaborates with physicians/APRNs, hospital staff, financial services on developing policies and procedures for the care of the patient.
• Facilitates patient in applying for financial assistance.
• Facilitates linkages to appropriate site of care to ensure necessary treatment.
• Coordinates and facilitates treatment objectives for the patient.
• Develops informational and educational materials for this patient population.
• Responds to and provides community education presentations.
• Proactively educates community-based medical providers of services available.
