Job summary

University of the Pacific seeks an Registered Nurse/Clinical Case Manager to deliver direct nursing care to students and coordinate care for high-risk cases across campus and community resources.

The role includes triage, immunizations, phlebotomy, health education, and care coordination; the successful candidate may relocate within California to serve within a university health setting.

Qualifications

• Current, unencumbered RN license in California.
• BSN or ADN with relevant clinical experience.
• BLS certification current.
• Up-to-date immunizations and titers.
• Minimum 3 years clinical nursing experience.
• Minimum 2 years in clinical case management.
• Proficient in case management principles and care coordination.
• Experience with electronic health records and standard office tech.
• Strong assessment, triage, communication, and documentation.

Responsibilities

• Perform nursing assessments, triage, and manage acute/chronic concerns.
• Administer medications, immunizations, phlebotomy, and treatments.
• Assist during exams; provide point-of-care testing and wound care.
• Educate on sexual health, mental health, nutrition and preventive care.
• Respond to emergencies and coordinate referrals.
• Document in EHR to regulatory standards.
• Support public health initiatives and immunization compliance.
• Contribute to outreach materials and staff meetings.
• Coordinate care across campus and external providers.
