Stepping Up Santa Cruz seeks a CalAIM Clinical Case Manager to coordinate care for ECM, CS, and CHW programs in Santa Cruz, CA. You will provide clinical consultation and develop individualized care plans with Case Managers to support members with complex behavioral health and medical needs.

This role blends office coordination with active community engagement, demanding initiative, dedication, and a genuine desire to improve lives.

Qualifications

• Active California clinical license (MFT, LCSW, or LPCC).
• Minimum two years of clinical experience with individuals with complex needs.
• Valid California driver’s license and access to a vehicle for member transport.

Responsibilities

• Conduct biopsychosocial assessments and clinical formulations.
• Develop and update trauma‑informed care plans with goals and interventions.
• Collaborate with Case Managers to support members with complex behavioral health needs.
• Perform intake assessments and create ECM and CHW care plans.
• Coordinate care with medical providers, psychiatrists, social workers, and community partners.
• Attend interdisciplinary team meetings and provide clinical consultation.
• Support crisis intervention, safety planning, and risk assessment.
• Document services in EHR per Medi-Cal and CalAIM standards.
• Assist with discharge planning for transitions from inpatient care.
