Job summary

Community Hope, Inc. is seeking a full-time Case Manager for the Bringing Veterans Home program in Parsippany, NJ. The role provides case management, eligibility screening, and linkages to housing and supportive services for veterans in need.

The position is hybrid, with field outreach and community-based visits, including travel in agency vehicles. A bachelor's degree and 1 year of case management experience with veterans or homelessness are preferred.

Qualifications

• Bachelor's Degree in Social Work, Psychology, or a related field.
• 1 year of case management experience, preferably with veterans or the homeless.
• Must possess a valid driver’s license, with a good driving record.

Responsibilities

• Provides case management and supportive services including intake, assessment, housing identification and stabilization, rent and move-in assistance (financial).
• Provides linkage and referral for mental health, substance abuse, legal, and other service needs.
• Provides eligibility screening to applicants and collects and maintains accurate and detailed records.
• Meets with participants in community settings and conducts outreach in shelters, encampments, and other locations.
• Transports veterans as needed in agency vehicles.
• Enters information into the electronic health record in a timely and accurate manner.
• Functions as a team member by covering shifts and duties as needed.
