Using an evidence-based approach focused on quality care management and data-driven medical decisions, Bluestone care teams collaborate to manage patients’ chronic conditions, address social determinants of health, manage transitions to and from inpatient settings, provide behavioral health support and more. Under our model of care, Bluestone patients experienced 21% fewer ER visits, 36% fewer hospitalizations and 41% fewer hospital readmissions compared to patients with similar conditions and complexities over the same time period.

Our care teams travel directly to patients who reside in Assisted Living, Memory Care and Group Home communities throughout Minnesota, Wisconsin and Florida and are supported by clinical operations and administrative colleagues who work remotely or at our corporate offices in Stillwater, Minnesota, and Tampa, Florida.

Position Overview:

The Primary Care Director, reporting to the SVP Head of Primary Care, provides strategic and operational leadership for the market-level implementation of primary care teams. This role is crucial for provider recruitment, placement, and overall performance across designated markets. The Director will foster seamless collaboration with clinical support and growth leadership teams to ensure optimal market performance and financial sustainability.

Schedule: Full time position. Up to 25% local travel is required for this position within the Minnesota Market. The remaining time is spent workingremote (work from home).

Territory: Minnesota Market

Salary: $110,000 - $140,000. Base salary will be commensurate with experience.

Other Compensation: Management Incentive Plan

Responsibilities:

Operational Management of Clinic Logistics (70%)
  • Optimize the deployment of Physicians and APPs to ensure efficient patient coverage across the market.
  • Collaborate with other market leaders to align support structures with patient care and provider needs.
  • Educate and support providers on best practices for managing and operating a Bluestone team.
  • Ensure providers understand their compensation model and performance expectations.
  • Conduct regular field visits to assess operations, provider performance, and external relationships.
  • Ensure all operations adhere to healthcare regulations, standards, and policies.
  • Own and manage the medical group market's Profit & Loss (P&L) to ensure financial sustainability.
  • Monitor revenue streams and collaborate with the market Clinical Director to achieve performance targets.
  • Oversee the provider compensation model, providing education and updates to providers.
  • Analyze financial performance metrics and implement corrective actions to address variances.
Business Development and Growth (30%)
  • Collaborate with the growth team, Sales Director, and Territory Managers on developing and executing growth plans.
  • Cultivate and maintain strong relationships with Assisted Living owners/operators, corporate partners, home health, and hospice agencies.
  • Represent the medical group in the market, serving as the primary point of contact for external stakeholders.
  • Identify and pursue new business opportunities.

Qualifications:

Education/Certification/Experience

  • Bachelor’s degree in business, finance, healthcare administration, or an RN/clinician with a background in clinic operations and/or care delivery. MBA preferred.
  • Five (5) or more years of leadership and healthcare operations experience.
  • Proven experience in business development and relationship management within the healthcare industry.
  • Must possess a valid driver’s license and minimum state-required liability auto insurance.

Knowledge/Skills/Abilities

  • Strong financial acumen with experience in budgeting, forecasting, and revenue management.
  • Exceptional organizational and logistical skills with the ability to manage complex operations.
  • Excellent interpersonal and communication skills for building relationships with diverse stakeholders.
  • Ability to lead, mentor, and motivate providers and staff in a fast-paced environment.
  • Thorough knowledge of healthcare regulations, compliance, and quality standards
