Job summary

UConn Health is seeking an RN Clinical Case Manager-Breast Nurse Navigator with oncology expertise to support breast cancer patients at John Dempsey Hospital. The role focuses on guiding patients through the care continuum, coordinating appointments and resources, and facilitating survivorship planning.

This full-time position requires MS in Nursing or a BSN with oncology experience, and includes responsibilities such as multidisciplinary conferences and data reporting to leadership.

Qualifications

• Master's degree in Nursing is required.
• Bachelor's degree in nursing with three years' oncology experience is acceptable.
• Breast Cancer Navigator certification is preferred.
• OCN Certification is preferred.
• Oncology navigation experience is preferred.
• Outpatient clinic experience is preferred.

Responsibilities

• Initiate contact with patients after suspicious mammogram or positive finding.
• Coordinate appointments and liaison with financial services for community patients.
• Educate patients about biopsies and schedule initial oncology visits.
• Navigate breast health issues and guide patients through care pathways.
• Log all new breast patients and malignancies in navigation logs.
• Coordinate Multidisciplinary Breast Cancer Conferences (Breast Tumor Board).
• Develop survivorship care plans within 1 year of diagnosis and within 6 months post-therapy.
• Report data to Breast Program Leadership Committee quarterly.
