A leading healthcare provider is seeking a Breast Health Navigator to support patients with breast cancer throughout their care continuum. This role focuses on assessment and intervention in collaboration with healthcare teams to enhance patient outcomes and provide educational resources, ensuring seamless navigation through the healthcare system. Ideal candidates will possess an Associate degree in Nursing and relevant experience in oncology care.
The Breast Health Navigator is a nursing practice role that serves patients with breast cancer and their families throughout the HealthPartners integrated health system. The Breast Health Navigator identifies the needs of patients with breast cancer from diagnosis throughout the care continuum, providing consultation and collaboration with the interdisciplinary team of providers (oncology, surgery, radiation therapy, and radiology) to assist these patients and their families in achieving their health care goals. Breast Health Navigator care includes assessment, intervention, and ongoing evaluation of the physical and psychosocial symptoms, care needs, and outcomes of patients with breast cancer; provision of patient education and counseling; design and delivery (written and presentation) of educational materials; identifying and assisting patients in obtaining resources within the healthcare system and community. This position will also support for other areas of the Breast Center as needed. Performs other duties as assigned
