Job summary

Golden Valley Memorial Healthcare is seeking a Breast Care Navigator (RN/Mammo Technologist) to guide patients through the breast health journey, coordinating imaging, biopsies, referrals and follow-up care. You will educate patients and families on diagnoses, treatment options, and survivorship resources, addressing barriers to care and supporting timely diagnoses.

Responsibilities

  • Initiate communication with patients following abnormal or suspicious breast imaging findings.
  • Facilitate communication among patients, physicians, providers, and interdisciplinary care teams.
  • Coordinate imaging appointments, biopsies, referrals, consultations, and follow-up services to support timely diagnosis and treatment.
  • Follow patients from initial abnormal imaging through diagnostic resolution, treatment, and survivorship planning.
  • Provide patient and family education regarding diagnoses, treatment options, recommended follow-up care, and available support resources in collaboration with the physician care plan.
  • Identify and address barriers to care, including scheduling challenges, transportation concerns, financial obstacles, and access to support services.
  • Monitor patient progress throughout the continuum of care and provide ongoing guidance and support.
  • Participate in multidisciplinary breast conferences, quality improvement initiatives, and community outreach activities.
  • Track patient interventions, outcomes, and performance measures to support program development and continuous improvement.
  • Serve as a central point of contact among radiology, breast surgery, pathology, medical oncology, radiation oncology, rehabilitation services, genetics, and supportive care resources.
