Job summary

Golden Valley Memorial Healthcare is seeking a Breast Care Navigator to guide patients through the breast health journey, coordinating imaging, biopsies, referrals, and follow-up care from diagnosis to survivorship. You will support patients and families with education and access to resources while addressing barriers to care.

The role emphasizes collaboration with radiology, surgery, pathology, medical and radiation oncology, genetics, and supportive services to improve timely care and patient

Qualifications

  • Current Missouri RN license or current Registered Radiologic Technologist with Mammography credentials.
  • Strong communication and interpersonal skills.
  • BLS certification required.

Responsibilities

  • Initiate communication with patients following abnormal or suspicious breast imaging findings.
  • Facilitate communication among patients, physicians, providers, and interdisciplinary care teams.
  • Coordinate imaging appointments, biopsies, referrals, consultations, and follow-up services to support timely diagnosis and treatment.
  • Follow patients from initial abnormal imaging through diagnostic resolution, treatment, and survivorship planning.
  • Provide patient and family education regarding diagnoses, treatment options, recommended follow-up care, and available support resources in collaboration with the physician care plan.
  • Identify and address barriers to care, including scheduling challenges, transportation concerns, financial obstacles, and access to support services.
  • Monitor patient progress throughout the continuum of care and provide ongoing guidance and support.
  • Participate in multidisciplinary breast conferences, quality improvement initiatives, and community outreach activities.
  • Track patient interventions, outcomes, and performance measures to support program development and continuous improvement.
  • Serve as a central point of contact among radiology, breast surgery, pathology, medical oncology, radiation oncology, rehabilitation services, genetics, and supportive care resources.
