Job summary

Located in scenic Honesdale, PA - Wayne County MH/ID seeks a Caseworker 2 (BCM) to deliver case management, coordinate services, and advocate for consumers. The role emphasizes treatment monitoring, crisis response, and 24/7 on-call coverage, with regular business hours and occasional irregular shifts.

Qualifications

• Six months of experience as a County Caseworker 1 or equivalent.
• Bachelor's degree with social welfare major or related credits and one year of social casework experience.
• Possession or near-completion of qualifying degree/credits within two months of graduation.

Responsibilities

• Provide case management services to individuals and families.
• Coordinate services across daily living areas and with providers.
• Monitor treatment progress and document care needs.
• Offer crisis response and 24/7 on-call coverage for blended consumers.
