Are you looking for an opportunity to advance your career while working with an extraordinary team? At Merakey, we put heart and soul into everything we do. We are seeking a Blended Case Manager to join our team in our behavioral health program in Lewistown, PA.

Earn $18.40/hour, plus mileage reimbursement

Schedule: Monday-Friday, 9 am-5 pm, plus on-call

Position Details

The Blended Case Manager (BCM) provides community-based services in Juniata, Huntingdon, and Mifflin Counties, assisting children, adolescents, and adults with a variety of behavioral health, medical, and community-living needs. The majority of time will be spent seeing consumers in their homes or in other community settings, with designated times in the Lewistown office.

Duties and Responsibilities
  • Help consumers build a strong network of community resources, supports, and services
  • Link consumers with needed services in their community
  • Advocate for consumers and their families, and collaborate to develop individualized service plans with measurable goals
  • Navigate complex systems such as special education, social services, and housing
  • Assist with access to basic living needs and skills like employment, housing, food, medical care, and recreation
  • Provide support for on-call crisis intervention

Qualifications

The ideal candidate will possess the following qualifications:

  • This candidate must have one of the following:
    • A Bachelor’s degree in sociology, social welfare, psychology, gerontology, anthropology, social sciences, criminal justice, nursing, counseling, or education
    • OR a Registered Nurse
    • OR a high school diploma plus 12 semester credit hours in relevant social sciences and two years of direct contact experience with mental health consumers
  • Must possess a valid PA driver’s license and reliable transportation
