Key Responsibilities:
  1. Assist clients in gaining access to medical, social, educational, and supportive services, reporting to the BETCM Supervisor.
  2. Organize and leverage natural supports like family and friends when needs cannot be met through these means.
  3. Coordinate with community resources and service providers; obtain specialized mental health and rehabilitation services as needed.
Overview of Roles & Responsibilities:
  • Help clients access necessary resources and support their needs.
  • Focus on outcomes that benefit the persons served, prioritizing natural supports and community resources.
  • Secure specialized mental health and rehabilitation services when required.
Core Functions:
  • Spend approximately 85% of time on consumer activities including assessment, advocacy, supportive counseling, and resource linking.
  • Allocate at least 15% of time to meetings and documentation such as service planning and reporting.
  • Devote at least 50% of time to direct client or collateral contacts outside the office.
  • Maintain 24/7 on-call responsibilities for emergency/crisis care, with a 15-minute response time.
Additional Responsibilities:
  • Perform other duties and special projects as assigned.
Requirements:
  • Bachelor’s or Master’s Degree in Human Services or related field, or
  • Registered Nurse, or
  • High School Diploma with 12 semester credits in human services and 2 years of experience, including 1 year in direct client contact, or
  • High School Diploma with 5 years of mental health care experience, including prior employment as an intensive case manager before April 1, 1989, or
  • Equivalent training and experience approved by the Agency and OHM guidelines.
