Job summary

Atlantic Health System is seeking a Birth Certificate Clerk to gather data for birth and fetal death certificates, enter details into the VERI, and ensure accuracy and confidentiality. You will educate parents, verify data with medical staff, and maintain meticulous logs for timely releases.

Responsibilities include monthly reporting, data integrity checks, and ensuring physician certification before discharge, with a goal of completing certificates within five days of birth.

Qualifications

• High School Diploma is required.
• Vital Statistic experience is preferred.
• Veri and DOH experience are preferred.

Responsibilities

• Create Birth Certificate by gathering data for forms and entering data into VERI with confidentiality.
• Educate married and unmarried parents on birth certificate process and paternity where applicable.
• Print basic reports to ensure database integrity and organize workload (Daily Delivery Log, Admit Checks, DC Mom, DC Baby).
• Verify data entry in VERI against medical records and correct as needed.
• Maintain on-site files and logs for birth certificate and fetal death records.
• Ensure physician certification and timely completion of birth certificates before discharge.
• Release birth certificates within 5 days of birth and manage subsequent releases.
• Minimize unresolved records in the VERI queue and resolve discrepancies daily.
