Job summary

San Jose Clinic is seeking a Part-Time Social Worker for its Anti-Human Trafficking Program in Gary, IN. You will provide direct social work support, coordinate care, and navigate resources for survivors or suspected survivors of trafficking.

You’ll collaborate with healthcare teams, shelters, and community partners to improve access to services, record-keeping, and program reporting while maintaining confidentiality and trauma-informed care.

Qualifications

• Master’s degree in Social Work required.
• 2–3 years healthcare/social work experience preferred; human trafficking experience preferred.
• Bilingual English/Spanish required.
• Demonstrated ability to work with trauma-affected, vulnerable populations.
• Knowledge of community resources, referral networks, and care-coordination practices.

Responsibilities

• Provides direct social work support to survivors or suspected survivors of human trafficking, including screening, care coordination, referrals, follow-up, and resource navigation.
• Coordinates medical, dental, behavioral health, and social-service referrals and assists with continuity of care.
• Works with shelters, case-management partners, advocacy agencies, and community organizations to facilitate services.
• Maintains confidentiality and documentation in accordance with HIPAA and clinic policies.
• Assists with outreach, trainings, data collection, and program reporting for the Anti-Human Trafficking Program.
