Job summary

Health Connect America, Inc. seeks a dedicated Targeted Case Manager to coordinate services for children, families, and adults within our care teams. You will develop service plans, connect clients with community resources, and document outcomes to support effective, compliant care.

You will collaborate with the treatment team, meet clients regularly, and participate in on-call support while ensuring regulatory compliance and quality standards across our multi-state footprint.

Qualifications

  • Bachelor’s degree in a human services discipline is required in Florida.
  • Master’s degree or equivalent credentials may be needed for Mississippi site.
  • Experience with children and families in case management or community resource capacity is preferred.

Responsibilities

  • Provide best practice, quality case management services and ensure documentation meets payer, state, and COA standards.
  • Develop service plans with input from the treatment team and Clinical Supervisor.
  • Connect families with community resources and follow up with clients and agencies to document referrals.
  • Complete documentation within established time frames.
  • Meet with clients weekly or bi-weekly for up to 12 months.
  • Attend bi-weekly Treatment Team meetings.
  • Act as liaison with professionals and agencies related to the client.
  • Recruit new referrals to keep caseload full.
  • Provide 24/7 on-call support as per program requirements.
  • Ensure compliance with all state regulatory bodies.

Responsibilities

The Targeted Case Manager provides best practice, quality case management services, ensure that all documentation is completed per Payer, State, and COA standards and requirements. Assist in marketing new referrals.

  • Communicate with customers, colleagues, and other individuals to answer questions, disseminate or explain information, and address complaints/concerns.
  • Develop service plans with the input of the treatment team and the Clinical Supervisor.
  • Connect families with needed and available community resources. Follow-upwith clients and agencies as appropriate to document use/success of referral.
  • Complete documentation within established time frames.
  • Meet with clients weekly or bi-weekly for up to 12 months.

Qualifications

State Specific Qualifications:

    • Mississippi:
      • Master’s degree in a human services, behavioral health, social work, counseling, or related field required.
      • Must meet payer-required credentialing standards, which may include:
        • Licensed Social Worker (LSW), or
        • DMH-approved Mental Health Therapist credential, or
        • Registered Nurse (RN)
      • Must have 2 years of experience in mental health care.
    • Florida:
      • Must hold a Bachelor’s degree in a human services discipline, including but not limited to: Social Work, Psychology, Sociology, Counseling, etc.
      • Must have experience working with children and families in case management or community resource capacity.
